Client registration area

 We like to offer a local service to the surrounding community. To make this possible we can only accept clients within the blue and yellow shaded areas. 

Please download the form below to begin registration for your pets.


Pre Registration form

For all new registrations please download and complete our pre registration form.

Please return completed forms in person or email hello@wavevets.co.uk

We will then contact you to make an initial health check appointment. 

Our standard appointments are £41.25. Complex and more involved appointments are £60 or second opinions are £84.70.

Once you have had your initial assessment registration can be finalised.
